Voluntary Participation: By signing this enrollment agreement you acknowledge that the training of this course is completely voluntary and you are of sound mind and body to participate in any of the activities within the program that could be mentally and emotionally demanding. You are not under medical, psychiatric, psychological or any other care that prohibits you from performing any of the techniques and do not hold Lightwork Seminars, Intl responsible for any mental, emotional, or physical reactions that could result from the training and activities of this program. Cancellation and Refund Policy for Training Programs and Seminars: 1. Lightwork Seminars, Intl will refund all money paid if the applicant is not accepted within 30 days. This includes instances where a starting seminar is canceled by the company. 2. If training is terminated after the participant enters the seminar, no refund will be issued. The participant has the option of rescheduling for a later date without making any further payments or transferring into another seminar of equal or lesser cost. 3. When the participant, without notice, fails to attend the seminar no refund will be given. 4. When the participant is terminated for a violation of a published policy which provides for termination, no refund will be given. 5. Refund requests made less than 30 days prior to the first day of training will not be honored. Refund requests made at least 30 days prior to the first day of training are subject to a $100 nonrefundable registration fee. Registration fees may be applied to any future training, at any location, in any format. Refunds or transfer requests must be made in writing to Lightwork Seminars either via email or standard mail. 6. Registration fees for Personal Growth, Spiritual Growth, or Introductory Seminars are nonrefundable. These fees may be applied to any future training, at any location, in any format. Transfer requests must be made 10 days prior to the first day of the seminar. Discontinued Programs: If the company discontinues a course or seminar after participants enter training, including circumstances where the company changes its location, participants must be notified in writing of such events and are entitled to a pro-rata refund of all monies paid unless comparable training is arranged for the by the company and agreed upon, in writing, by the participant. A written request for such a refund must be made within 30 days from the date the program was discontinued or relocated and the refund must be paid within 30 days of receipt of such a request. Termination by the Company: A participant who fails to maintain satisfactory progress, violates safety regulations, interferes with other participants’ work, is disruptive, obscene, under the influence of alcohol or drugs, or does not make timely payments, is subject to immediate termination. In such cases, no refund with be issued. Cancellation of Classes: The company reserves the right to cancel a course or seminar if the number of participants enrolling is insufficient. Such a cancellation will be considered a rejection by the company and will entitle participants to a full refund of all monies paid. Notice to Buyer: Do not sign this agreement before you read it or if it contains any blank spaces. This is a legal document. All pages of this agreement are binding. You are entitled to an exact copy of the agreement, catalog, and any other papers you may sign. Cancellation of Contract: If you have not started the seminar, you may cancel this contract by submitting written notice of such cancellation to the company at its address shown on the contract, which notice shall be submitted not later than midnight before the seminar begins. In the event of dispute over timely notice, the burden of proof rests on the sender. Unfair Business Practice: It is unfair business practice for the company to sell personal information without the signed written permission of the participant. Agreement is Binding: This agreement will be binding only when it has been fully completed, initialed, signed, dated, and submitted by the participant prior to the time instruction begins. Changes in the Agreement: Any changes in the agreement will not be binding on either the participant or the company unless such changes are acknowledged in writing by an authorized representative of the company and by the participant.
